Families are often anxious about whether their loved one will receive the right support at home. Amari Care Services has a strong working relationship with NHS services, specifically the only branch in the group offering nurse-led care, PEG feeding, stoma, wound and catheter care. This relationship reflects the clinical standards, reliability and professionalism that define everything we do.

Whether your parent needs support following hospital discharge, ongoing Continuing Healthcare at home, or short-term reablement care, our team is ready to deliver safe, compassionate care that meets NHS standards, often at short notice of referral.

Hospital Discharge Support

Our hospital discharge care service is designed to make the transition from hospital to home as smooth and stress-free as possible. We work closely with NHS discharge teams to understand your parent’s clinical needs, medication regime and mobility requirements before they even leave the ward.

How We Support Hospital Discharge

  • Responsive care capability. We work to begin care promptly after referral, helping your parent return home sooner and reducing prolonged hospital stays while care packages are arranged.
  • Coordinated handover. We work directly with NHS discharge teams to understand each patient’s clinical needs, medication requirements and mobility support before they leave the hospital.
  • Tailored care plans. Every discharge care plan is written specifically for the individual, based on clinical notes, family input and our own assessment. We do not use generic templates because your parent’s needs are unique.
  • Consistent carers. We assign familiar carers to each patient from day one, so there is continuity from the first visit onwards. This reduces anxiety and helps your parent settle back into their home routine.
  • Digital clinical records. Every visit is logged on our secure platform with accurate observations. If anything changes in your parent’s health, we flag it immediately to our office and to NHS teams if relevant.

NHS Continuing Healthcare

Some patients qualify for NHS Continuing Healthcare (CHC)—fully funded care at home for individuals with complex or ongoing health needs. If your parent has been assessed as eligible for CHC, Amari Care Services can deliver the care package on behalf of the NHS. Whether your care is privately funded or funded through NHS Continuing Healthcare, the standard of care you receive is exactly the same. We do not operate a two-tier system.

What Continuing Healthcare Covers

CHC is available for people whose primary need is health-related. It can cover a wide range of domiciliary care services, including:

  • Personal care—washing, dressing, toileting and mobility support
  • Complex care—medication management, wound care and clinical observations
  • Specialist dementia care with trained, experienced carers
  • Overnight care—waking or sleeping night support as needed
  • Live-in care—24-hour care in the comfort of your parent’s own home

Every client receives fully trained carers, a personalised care plan, and access to our digital care platform. How We Maintain NHS Standards

Qualified, Trained Carers

Every member of our care team completes the Care Certificate (16 modules) before their first client visit. We fund NVQ Level 2 and Level 3 qualifications in Health and Social Care for all carers. All carers are Dementia Friends certified and receive ongoing training through digital learning modules and face-to-face supervision. Enhanced DBS checks are completed and funded by Amari Care Services for every carer before they start.

Digital Care Records and Clinical Accountability

Our fully digitised care platform (OpenPass) ensures that every visit is recorded accurately, care plans are updated in real time and clinical observations are flagged immediately. This level of documentation is essential for NHS-referred patients, where clinical accountability and accurate record-keeping are paramount. Nothing gets lost. Nothing gets forgotten.

Professional Oversight and Care Procedures

Our Registered Manager (Stef Clark RMN) maintains direct oversight of all care delivery, with regular spot checks, supervision sessions and care plan reviews. For clients with complex clinical needs, we coordinate directly with district nurses, GPs and NHS teams to ensure joined-up, seamless care.
